  • Home Health care consists of Nursing Care, and/or Therapy services to recover from illness or injury or chronic conditions management at “HOME” among your family and friends.

    Home is the place where you live. It can be a personal residence, assisted living facility, or an independent living community or any place you call Home.

  • Individuals in need of medical assistance following surgery of illness, hospital, or inpatient facility discharge, or need assistance to manage a chronic condition.

    An individual who requires considerable and taxing efforts to leave home due to current illness.

  • Home Health visits are commonly covered by Medicare or private insurances if individual has one. Once we receive the referral, we will verify with the insurance to determine the coverage. If you have any questions regarding coverage, please contact the office for further assistance.
